Alamak forgot to take some pic during Chrismas Eve Diner.. everyone was buzy ... my dad buzy kemas2 rumah.. my bro pasang the lights and candle @Grotto.. mr, my mum, my sis and my sister in law buzy @kitchen... i bake the coklate moist cake , make some pudding and corn soup + seafood.. my mum.. yeahh.. cook the Roosted Chicken, my sis in low cook the fish.. wahhh.. siok oo that time.. mcm mau revine balik that time...

Advent Season Liao... \(^^,)/
Advent comes from the Latin adventus, meaning 'coming.' The importance of the Advent season remains to focus on the coming of our Lord. So, this is the time we preparing ourself for His second coming. On one hand, the faithful reflect back and are encouraged to celebrate the anniversary of the Lord's first coming into this world. We ponder again the great mystery of the incarnation when our Lord humbled Himself, taking on our humanity, and entered our time and space to free us from sin. On the other hand, we recall in the Creed that our Lord will come again to judge the living and the dead and that we must be ready to meet Him.  A good, pious way to help us in our Advent preparation has been the use of the Advent wreathe. The wreathe is a circle, which has no beginning or end: So we call to mind how our lives, here and now, participate in the eternity of God's plan of salvation and how we hope to share eternal life in the Kingdom of Heaven. The wreathe is made of fresh plant material, because Christ came to give us new life through His passion, death, and resurrection. Three candles are purple, symbolizing penance, preparation, and sacrifice; the pink candle symbolizes the same but highlights the third Sunday of Advent, Gaudete Sunday, when we rejoice because our preparation is now half-way finished.The light represents Christ, who entered this world to scatter the darkness of evil and show us the way of righteousness. The progression of lighting candles shows our increasing readiness to meet our Lord. In all, during Advent we strive to fulfill the opening prayer for the Mass of the First Sunday of Advent: 'Father in Heaven, ... increase our longing for Christ our Savior and give us the strength to grow in love, that the dawn of His coming may find us rejoicing in His presence and welcoming the light of His truth.' Penyakit ne.. common diasese d Malaysia.. bila Dr telah mengesahkaan diagnosis ini.. biasa orang akan cakap 'ala.. besa la sudah tua.. semua penyakit pun datang '.
Bukan semua penyakit datang di hari tua.. muda- muda pun kalau tidak mengambil tahu tentang jagaan kesihatan... tidak mustahil akan mendapat penyakit ini.... jeng jeng.. penyakit yang di maksudkan adalah... Hypertensive Emergencies... last week, 1 of my patient newly diagnosed Hypertensive Emergencies.. She is very shock, why she got this disease even she is still young (28).
Hypertensive Emergencies
A hypertensive emergency is severe hypertension with acute impairment of an organ system (especially the central nervous system, cardiovascular system and/or the renal system) and the possibility of irreversible organ-damage. In case of a hypertensive emergency, the blood pressure should be substantially lowered over minutes to hours with an antihypertensive agent.
During a hypertensive emergency uncontrolled BPs lead to progressive or impending end-organ dysfunction; therefore, it is important to lower the BP aggressively. Acute end-organ damage may occur including neurological, cardiovascular and other. Some examples of neurological damage include hypertensive encephalopathy, cerebral vascular accident/cerebral infarction, subarachnoid hemorrhage, and intracranial hemorrhage. Cardiovascular damages include myocardial ischemia/infarction, acute left ventricular dysfunction, acute pulmonary edema, and aortic dissection. Other end-organ damage include acute renal failure or insufficiency, retinopathy, eclampsia, and microangiopathic hemolytic anemia.Also, severe blood pressure leads to problems in the eye such as retinopathy or damage to the blood vessels in the eye.
It is also important that the blood pressure is lowered not too abruptly, but smoothly. The initial goal in hypertensive emergencies is to reduce the pressure by no more than 25% (within minutes to 1 or 2 hours) and then toward a level of 160/100 mm Hg within 2–6 hours. Excessive reductions in pressure may precipitate coronary, cerebral, or renal ischemia. The diagnosis of a hypertensive emergency is not only based on the absolute level of blood pressure, but also on the individual regular level of blood pressure before the hypertensive crisis. Individuals with a history of chronic hypertension may not tolerate a "normal" blood pressure.
Several classes of antihypertensive agents are recommended and the choice for the antihypertensive agent depends on the cause for the hypertensive crisis, the severity of elevated blood pressure and the patient's usual blood pressure before the hypertensive crisis. In most cases, the administration of an intravenous sodium nitroprusside injection which has an almost immediate antihypertensive effect is suitable but in many cases not readily available.
